Does this one extinguish when its tilted to the side like mine does?

Yes. When burned horizontally, its extinguish: link . But not always. Sometimes it only rectifies.

Voltage change which normally doesn't affect a magnetic choke lamp because of the voltage spike created on every zero volt crossing of the waveform?

The problem is caused by hydrogen impurities that increases the ignition voltage and restriking voltage after every mains half-cycle, causing the arc to become unstable in non vertical position.
