Osram OS13 metal cannister starter, listed for 3ft 30w, 4ft 40w and 5ft 80w tubes.
Made in England, working fine. Came in [url=http://80.229.24.59:9232/gallery/displayimage.php?pid=6222][u]this[/u][/url] GEC 5ft fitting
